Hi everyone,
I've been taking concerta XL 36mg for 3-4 months now, it's been okay but I've still had quite a lot of trouble with being unable to concentrate.
My doctor prescribed me 10mg of instant-release medikinet, to take in the evenings when the XL wears off, about 6 weeks ago. I've noticed that I'm usually able to concentrate really well as soon as I take these tablets, until long after the XL tablet has worn off.
I'm wondering why the normal tablets are so much more effective than the extended release, could it be because the XL dose isn't high enough to have the right impact? With methylphenidate the case seems to be that below a certain threshold, not much seems to happen... This seems like the opposite of other drugs (eg I feel like there's a bigger difference between 0 beers and 1 beer, than there is between 1 and 2, and the same with caffeine)
As far as I know, with concerta the next step up from 36mg is 54... I remember one time I took a 36 and an 18 I had left, it felt a bit unpleasant and different from when I take the 10mg tablets I've been prescribed. So I don't think I'd want to go up to 54mg if there's no dose in between
If the XL tablets aren't doing much, might I be better off not taking them at all, and just taking a few 10mgs as and when I need them?

Hi goldfish Due to delivery system some people prefer IR over XL although XL is more convenient as it's only taken once a day. Maybe you could ask your Dr if you could continue titration on IR. . . although you'll have to try to remember to take it 2 or 3 times a day. 10mg IR is equivalent in strength as 36mg XL, just not as long lasting. .

I take 2-3 36mg tabs of Concerta XL per day. For me, this is how it works. It lasts 10 hours for me. So if I wake up late or don't have to work, I take 2 and I'm done for the day. If I have to go out in the evening, work late, or wake up super-early, I take my 2, and then at around 2PM (or later if I face a super-late night), I'll take another. There is no evening drop with that scenario for me. My doctor - wrongly - informed me that instant release is not even available on prescription, and he treats me like I'm some junkie who gets off on them. I don't even like the effect. I'd much rather not take them. But I'm useless without them. So that's how I convinced him to get over that evening or afternoon drop, by giving me the dose in 2/3 increments. My daily dose is 109mg and I get it in 3x36mg pills so that I can take as I described above.
