I have recently been diagnosed with ADHD via the NHS, and have started medication.
I'm currently on Methylphenidate Instant-release 5mg TID, and have been for approximately a week. Although it effectively calms that "brain noise" common with ADHD, it also causes me to be extremely fatigued, to the point where I am more tired/less productive taking the medication than I ever was before! Has anyone else had similar effects with methylphenidate?
Also I think this would be a good place to ask for general medication experiences, as it is very hard to find information from adult ADHD patients, especially UK based ones! Do you find better improvements on IR or XR medications? Do you personally respond better with methylphenidate or dextroamphetamine?

It could be that it's making it harder for you to sleep, making you tired... But 5mg isn't loads and probably wouldn't keep you awake unless you're taking it in the evenings
I do remember when I started taking methylphenidate (18mg XR which is roughly equivalent to 5mg IR) I didn't notice any improvement for the first 2 weeks and got really upset that it didn't seem to be working... I think it took my body time to get used to the medication and for me to reevaluate my expectations of it
I'm now on 36mg XR (lasts around 8-10 hours) then I take 10mg IR around 6pm. I find that I get more done after taking the IR (even when the XR has worn off)

Hi gman Everyone responds to medication differently. I also felt very tired on methylphenidate (mph) when I started and when I increased dose. I had the best sleep I ever had for the first couple of weeks ! If you want to find UK patient experiences of meds, there's absolutely tons of stuff on this forum if you have a root around. IR vs XR. . .you can get slightly different effects . .but mainly it's down to whether you prefer to take one tablet in the morning, or are able to remember to take several during the day. It's more likely you'll experience a crash when XR wears off . . hence why goldfish takes an IR in the evening. I know people who have had good and bad experiences on both mph and lisdexamphetamine, it's very personal and it wouldn't be helpful or right to say one is better than the other. I've tried both. . .mph IR didn't do much beyond making my sensory issues worse and I always forgot to take them . .mph XL made me tired, didn't improve my concentration, and I experienced awful crashes and rebounds, but I found my mood was more stable. Lisdex made me feel less anxious and like I'd had a good night's sleep, even when I hadn't, but my concentration didn't really improve and it sent my BP dangerously high, so I had to stop. My experience is just my experience. .everyone is an Individual and responds differently, positively or negatively . .only you can discover what meds are right, or wrong for you.

Hello I'm on 20mg methylphenidate (ritalin) 3x daily. It's okay, I didn't think I was getting much out of it. What it does seem to do is help get my ideas/thoughts out of my head more easily and I'm better able to either vocalise or type them with less effort and with a bit more fluidity.
(Currently enjoying a birthday tipple!)
Apparently 60mg is a relatively high dose, it doesn't do a great lot to me. Apparently the average neurotypical would be hyper on that much!
Battle on. Find the right dosage that helps you get through the day. I never got the great sense of clarity that other people seem to get, but it does seem to help me.
