Hi I'm new here.
I'm 48, 8/9months in on concerta xl 72mg. I have severe combined internal adhd.
Initially relived about diagnoses as it gave me answers to my life time of struggle and always feeling different to everyone.
I took to the meds pretty well with very little side effects and saw some initial improvements ie: flash temper, not as stressed, bad moods more motivated and some focus. 6 months down the line, I feel quite down and overwhelmed by life and the whole thing, my melancholic nihilist attitude is rearing its head again. Its as if I have realised how much catching up I need to do in life, home and work. My lists are now huge and its as if I'm overwhelmed by what needs to be done.
Unfortunately my GP and local health board are useless with adults, I've had to pay privately for my psychiatrist who says she feels the meds are working and its going to take time.
My wife and teenage kids say home life is a lot better ( no horrible frustrated flash tempers and more reliable) but I feel more shit than pre diagnoses.
Any one out there with a similar story or advice, pointers or suggestions?
Thanks

I would definitely give Elvanse a try if I was you. If you have a big to do list, that’s the tackle you want.
Good luck

Thanks. That's whats been suggested by Dr. 1 more month and I will probably swap over. Its all the titration and getting used to the meds is stalling me. Are you still on Vyvanse/elvanse Addz, what else have you used?

You've used the word melancholy and nihilism but these are not 'symptoms' of ADHD. Did you always have these symptoms or have they presented themselves since going to 72mg on Concerta?
My 'gut' says Concerta is only effective for those who benefit from the 18/36mg range.
You might wanna consider trying Elvanse sooner rather than later.
Whatever drugs you do or don't take, help your body get rid of excess shit by sweating/exercising at least once a week. Some of us can't get rid of the basics, let alone the extra waste imposed by stimulants.

Yes OP I also tried Concerta & Strattera. Steady titration is all find and dandy if you (or your wallet) are not suffering in the meantime! Stay clear of Strattera and jump on this Elvanse express train, fuck knows where it’s going...but it’s going

I would definitely give Elvanse a try if I was you. If you have a big to do list, that’s the tackle you want. Good luck Im on 72mg (54+18) concerta XL daily. I started on 18 daily then went to 36 then after 6 months started 18 and 54. I had terrible tiredness during the day. I could sleep at night for 7hrs straight. That seemed to ease over a period of time. 2 years down the line has my symptoms got better. Yes my implusivness has given me a chance to think but it doesnt go straight away. Think of your brain has been doing behaviour the same way since you learned to talk it isnt going to go away. Ive 30 years to try undo. Also i have found any caffeine or alchol accelerates your absorbtion rate so you i find i talk more and impulsive comes back worse. Some colouring in foods and hard to track food companies wont say which E numbers are in their food Concentration is better but i can zone in and i dont lisen Its been better on them Angry person happens sometimes but because people dont lisen to me ... its like they dont lisen either
