Hi all I was diagnosed with ADD 18 months ago and I was started on concerta 18 mg to start with but within 6 months I was at 72 mg which seemed to work for me I had a little aniexty at times which some diazepam helped with I had the usual not eating great are sleeping great but at 6 months it had all settled and I was feeling normal again , I would get up at 6 take my dose and get to 8 are 9 that nite out of them , thing is over the last few months things have been getting progressively worse meaning wearing of quicker now I’m taking them at 6 and 3 hours later they have word of , I’ve tried playing with the dose going as high as 136 and still not working, what is happening and can anyone suggested another medication that mite work for me as I feel at the moment I’m back at square one if not worse , I’ve an appointment on 17 dec with my phycarist , I guess I’m jus looking some people’s experiences, what other meds I could suggest, I always seem to have brain freeze at these appointments Thanks for listening, any help / info so greatly appreciate

It's annoying when they seem to stop working.
That sounds a pretty high dose for Concerta? I guess Elvanse might be worth a shot?

Hi brownerr5791 There are various options . . you could stick with concerta and take a second tablet later in the day, or see if a top-up of instant release methylphenidate works. You could change medication to lisdexamphetamine (Elvanse) which generally lasts a bit longer than concerta. Both rely on metabolism so can last different lengths of time in different people. If there was an ideal medication which worked the same in everyone, there would only be one. . . Also, consider if your current meds are *actually* wearing off quicker, or if you’ve just got used to the way you are while taking them.

Update , things have improved a bit I was taking 72mg at 6 in the morning and at 10 I was taking a 54 , now I’m taking 108 at 6 in the morning and at 10 I’m taking a 18 mood and concentrating have improved slightly and I’m getting to 4 in the afternoon with that formula though not perfect , I also have rhumitoid arthrites which has flared up this week so the doctor give me 30/500 co codamol , I taking 4 a day and I don’t no what it is but the mix of them and the concerta has been like when I first was prescribed concerta ( everything has calmed I can get things done I can talk to people I can consentrate , I can even go to Tesco’s and do shopping and accuatlly talk to people which of the last 6 months has been a big no no , does anyone no what has happened and what is going on Thanks pb

Could be any number of reasons, I suppose? Does pain medication reduce blood pressure? That might be one reason, possibly? My very basic understanding of our blood vessels is that they are like hose pipes with tiny slits cut in them throughout. Stuff moves out of the blood via these slits. If you turn the hose pressure up too much, those slits become useless, I guess? Far fewer nutrients will be able to cross over to the organs, perhaps? Whatever the cause, make it your GP or Psych's problem and let us know the outcome

Update , went to see my phyc today and I explained everything to him as above , he could not change my adhd meds as concerta is the only licienced drug for adult add in the uk , I explained to him about the painkillers, he wasn’t sure what was going on so he put me on amyitriptline along side the concerta as the work on the same areas of the brain and he said the amytriptline will act as a booster to the concerta plus he said I will get a better night sleep and he said I will notice a difference by 2 weeks , so I’ll give it a whirl and keep updated

Your Dr’s wrong!! The only ADHD medication licenced specifically for adult use is Elvanse.
